I typically do not add anything else here. What is the perception by adcoms if an applicant doesnt put anything here?

Is there a downside to leaving it blank or am i overthinking this?

Your neurotic mind is probably talking to you.
It's a good opportunity to explain defects in academic record, or simply add a hobby and make yourself interesting.
I don't think adcom will look down on you if you don't add anything in.

But generally , it's good to have something extra about you that make you more interesting. It won't hurt you if you don't have anything.
I don't have anything but still receive an II.
