Is it not helpful to tell your number 1 program they are number 1?

Is it not helpful to tell your number 1 internal medicine program that they are number 1 after you finish with all your interviews?(around this time)

All depends on the program. Our applicants are ranked before they get to the airport and the list isn’t touched again. It would make absolutely no difference for us.

There are some programs and PDs that will fine tune their lists over time and there’s a small chance it would make a difference for one of those.

Be honest though. Residency is short and soon you’ll potentially be looking at fellowships and/or jobs and people will remember if you lied to them. Only send those notes if they are true.
 
At a panel of some of the PDs at my program they recommended sending an email to your #1 choice because it has the potential to help a little bit or at worst not change anything. Some programs rank right after the interview but many programs have indicated that they have final meetings in the winter where they make adjustments to the list. From a risk/benefit standpoint I don’t really think it could hurt you at all, unless of course one were to send multiple “#1” emails (but I don’t really think people actually do that).

So do you only have 1 interview day at your program or can applicants move down on the list if other are interviewed later and ranked higher up? Sorry of this is a dumb questions, I'm pretty clueless when it comes to how residency interviews/rank lists work.

We do 2 days back to back, then rank. Rank meeting happens 30 minutes after last interview. After that meeting it’s never touched again.

That’s just one program though. Others do it differently I’m sure.
 
There are literally a million threads about this exact topic that you can easily search for. Almost everyone says that it's unlikely to hurt, but at most programs you shouldn't expect it will help because either 1) They don't care and are just going to rank based on who they think will be most qualified, or 2) They assume that a lot of applicants will send similar messages to all of their top programs.

This question gets asked each year because everyone wants to believe that's "something more" they can be doing other than sitting on their hands and waiting for match day, or worse, they're having FOMO that everyone ELSE is doing something. There probably isn't anything you can do at this point, but if it makes you feel better go ahead and send that love letter.
