I'll be applying for my residency programs in 2012 and by then will hopefully have enough clinical experience. I'm an IMG and will look to do externships next year.

My research experience is pretty bleak. I've so far published only 2 papers, one of which was rejected.

My question is: Can i do my Phd after I complete my residency program despite not having much research experience? Or is this too difficult a task?

I'm no expert but I know people have completed a PhD in conjunction with their residency, it just extends the residency. I'm not sure this this would only be doable at certain programs or in certain departments.

I'm sure there are many, many more difficult tasks than this plan but still sounds sorta rough.
