As an IMG currently studying in India, I have heard some students venturing off into Emergency Medicine.

The question I pose is whether or not your weekwork is satisfying as I have heard horrifying stories from some students week after week.

Is there any difference in satisfaction from the following:
1. EMS
2. Hospital setting/Emergency Department
3. Private Practice
4. Combined Residency
 
Usually the people who go into emergency medicine are satisfied by it. Your question is rather broad and vague, so it's hard to know exactly what you're asking. It's not everyone's cup of tea, so usually students either end up loving or hating their EM rotations.
 
EMS can be pretty satisfying but realize that as a doc your role in EMS is predominantly teaching and administration. The least satisfying part of EMS that I have found is that you get into it to work with the best, brightest and most motivated and you spend all you time dealing with the problem children.

The "Hospital setting/emergency dept." is not an optional thing. I don't know if it's satisfying but it's where we work.

Do you mean private practice as opposed to academics? Either private practice or academics can be satisfying, or not. Depends on the person, ie. what you want and what you like to do.

I don't know much about combined residencies but there are different combinations to do different things. Wheather any of them are satisfying or not depends again on the person.
