Is Patient Consent needed for case report ?

I am currently at a brand new DO school so I have no one above me to ask. Do I need patient consent (verbal or written) in order to use them for a case report? I would just be going through their chart and extracting the pertinent parts of their case. Obviously I would not extract any identifiable information like name or DOB. I just want to ask before I start doing any of this. Any input would be much appreciated. Thanks.

Never seen it done by any of the clinicians I've witnessed giving a case report
 
Depends on the context. There are some journals that actually force you to have a signed consent even if you don't have identifiers. If you're presenting a case without identifiers, most of the time the answer is you don't need signed consent. My advice is research the journal or professional organization you'll be talking to and always follow HIPAA

What about for a poster presentation? I don't think I have enough time to try and publish but I can manage to do a poster presentation. I'm just kinda pressed for time with 3rd year clerkships and I just want to add as many clinical vignette poster presentations to my CV before ERAS opens up next year since I literally have nothing on my CV.

Probably doesn't matter, but again, it might depending on the group hosting the poster presentation. They should have guidelines online for case reports.
