I say yes, definitely send a thank you note. It shows maturity, professionalism, perseverance, and it is proper etiquette. It can't hurt. Dental school is extremely competitive and anything you can do to set yourself apart from the sea of other qualified applicants is worth doing!

In my personal case, after sending a thank you note, I received a hand written note of thanks and encouragement from one school's assistant dean. So, I definitely got noticed and ended up getting accepted to that school.

should we send the note to the admissions review committee or the dean of admissions?

I don't know the right answer. I sent notes to anyone I had significant contact with throughout my interview day (faculty interviewer, student interviewer, admin facilitator) and the dean of the school. This was probably overboard, but GD it, I was determined to do everything within my power to ensure I got into d-school.

Is thank you card enough? or we have to write a letter and send it with the card?

Thank you

It looks like it is proper to send a thank you letter, not a hand written note (as I did, oops). You can google "thank you letter dental/medical school" to get some ideas on what to write. Here's a start, though it is for med school, I think it applies equally to dental school: http://www.admissionsconsultants.com/medical/ty_notes.asp
