Issues facing EM

OK all, help me out. Had an interview today and got this question: "There are many issues facing our specialty today. What do you see yourself doing to address them during your career?"

I just sat there. I thought I was pretty knowledgable about EM as a field, but I had no idea how to answer this question.

What ARE some of the major issues facing EM today???

BTW, I also got this one: "Make up a question to ask yourself that I haven't asked you." How the heck do you do that?

There are tons of things you could throw out for this. Wound care, chest pain obs units, stroke centers, public health, reimbursement, political involvement, etc. You just have to pick something and be able to discuss it.
