J1 visa after residency?

I am presently on J1 visa, doing psych residency.

I would like to do fellowship after my 4 years of residency.

Hypothetically, if I match into a fellowship, do I have to submit a new J1 visa application/request a new Statement of Need from my new hospital, or can I just renew my current J1 visa?

For fellowship, it will be considered continuation of training under same J1 visa (7 years) and sponsorship by ECFMG. However, for this you will need to submit another SON to ECFMG.
 
SON is gotten from home country or country of last residence NOT the hospital.
You don't do anything its under same J1 visa. You just get another SON for fellowship.
