Step I Kaplan Qbank Media Question

Hi everyone,

I just started utilizing my Qbank today, and I got a question on a heart murmur. The media shows a generic chest auscultation with the stethoscope in one place but also three other locations with + signs in a circle. What do these represent/mean in terms of the question? Is it only where the stethoscope is shown that is of concern and the +'s indicate sort of where you are in relation to other auscultation points? Thanks in advance 🙂
 
I just finished all of my step 1 Kaplan Qbank today and I think I never saw one of the chest auscultation videos where they actually switched auscultation points. The other points are just so you can accurately pinpoint exactly WHERE you are listening, so that there is no confusion as to where you are (or aren't) auscultating.

You are supposed to move the stethoscope around. They represent the aortic, pulmonic, tricuspid, and mitral valve areas. You click on each one to hear the heart sound. Eventually, there will be a murmur at one site. Then you answer the question after having determined the murmur based on systole/diastole and location.

On Kaplan's Qbank, the auscultation parts are essentially a video file that only has a still frame of a chest, but with audio playing. So sort of like a video (it's the same media player that plays videos of patients, clinical signs, etc.). It isn't really interactive; you can't "move" the stethoscope. They always only show you the one area where the murmur/gallop/sound is.

So if I move my cursor over the other auscultation points, will the sounds change? Also, is this how these questions appear on the actual Step 1 because UWorld does not have this format? Thanks in advance 🙂

Not on Kaplan's Qbank :laugh: I don't know about other question banks...

I don't know because I still haven't taken my test (10 days) but I don't think you can change the auscultation points on the real test either. It's more technologically complex (interactive instead of just playing a video/audio file). They don't want to test whether you know how to put together different auscultatory findings - they want you to identify ONE obvious abnormality in one location. But like I said, I wouldn't know. 🙂
 
Just took my test and wanted to post here to point something out. My previous post said that I didn't think you would get the chance to change auscultation points on the test. I hadn't heard about that before, but I post here to clear things up: On the real test I did get two questions with associated auscultation. It was 100% interactive and you could change between the major four auscultation points. You could also see the patient's neck (to look at the carotid upstroke in relation to the sound) and there was also a small stopwatch/clock which I suppose is for timing heart rate or the duration of sounds or something.

One of them was aortic regurgitation (blowing decrescendo diastolic sound mostly over the apex) and I don't remember well but the other one was mitral stenosis. It was straightforward "what does this patient have".
