What are some difficulties an orthopaedic surgeon runs into during the revision of a cementless tibial baseplate implant? What is the most difficult part of the surgery and do you have any recommendations to solve these problems?

Briefly
Why does it need to be revised? Infection? Osteolysis? Periprosthetic Fx? Malpositioning? Ligament imbalance/instability?

How much bone loss/any deformity/need for augments or custom implants

The surgeon has to diagnose the why and then come up with a good preop plan to fix the problem
 
My question was more focused on issues during the surgery. Is there a lot of difficulty cutting through the implant to remove it? How do surgeons typically deal with the heat and metal flakes generated?

You don't usually have to cut the actual implant. You remove the implant from the bone-implant interface using osteotomes (chisels), and other tools.
