lameo pre-med question: but pertinent damn it! (on surgical residencies)

I'm applying to medical schools now, and have a number of interviews. I am relatively sure I want to pursue some surgical speciality. So in typical lame pre med fashion, I started paroosing match lists. Frankly they mean very little to me at this point because I have realized I don't really understand how this whole "residency" dealy works:

1. Are you applying out of medical school for a residency or an internship. Internship is just the first year of residency right? So what is "surgical preliminary", do you complete that and then sub specialize from there?

2. Is "general surgery" a sub speciality? or do you go on from there to do a fellowship in some random area that your interested in?

3. It seems like there are surgeons who specialize in all sorts of strange procedures, and what residency do they begin in? What for instance, would a kidney/liver transplant surgeon match in?
