I haven't received my sub-I grade yet and decided to hold off on sending my transcript for these past few days. I'll be getting the grade and sending the transcript in the next day or two. How will this negatively affect my application? Has the screening process already started? I am applying to internal medicine and was assuming programs won't start looking at apps until after the MSPE is sent on Oct 1st. The remained of my app was submitted Sept 15.

Screening starts immediately. Most programs don't wait for the MSPE to make initial categorizations of applicants--that's just the last piece of information needed to make decisions for applicants that don't clearly rate a "yes" or "no" based on scores/transcripts/letters/activities etc. Waiting isn't awful--in the long term it doesn't affect the outcome of whether you'll be invited to interview--but it isn't helping you either. (Exception: if your IM clerkship grade was mid-tier and you expect your Sub-I grade to be exceptional)
