You might want to take a look at the sticky FAQ on fellowships at the top of this forum. I believe it organizes all fellowships by competitiveness, and includes other relevant facts.

My vote would be:

1. endocrinology
2. rheumatology
3. infectious disease

There is also a fellowship in general internal medicine for those that want to do academics. I bet that is not competitive either.
