Letter of intent for shadowing

Some people told me that a certain clinic accepts shadowing (maximum 2 days) if I send them a cv with a letter of intent.
What should I write in a letter of intent for shadowing?
Can someone give me an example or some tips on what to write there?

Are you asking how to write a letter of intent for a shadowing position at a clinic?
I don't know any clinic that strict with who they want shadowing them... If you're looking for shadowing hours, just politely walk into a dental office or ask your general dentist or specialist.

This place asks for a letter of intent and a CV. Because it's part of a hospital, not a private practice. So, I don't know what people write in a letter of intent
I already did the calling, going in person to dental offices. Visited more than 25 office ended up with shadowing 5 different dentists. I still need more hours to hit the 100 hours.

Why not continue shadowing one of the 5 dentists you've already shadowed with to get the hours? I think a letter of intent + CV just to shadow somewhere else is more work than necessary just to get your shadowing hours up.
