Can anyone pls help me with the format and what all should be mentioned in the letter of observership.
Its urgent .

Some schools have a form they want the dentist to fill out while for others a letter from a dentist will suffice.

Generally, this document includes names of the doctor and the applicant, type of the practice, number of hours you observed, and duration in which observation hours took place (you did 40 hours in the span of one week or several weels). Dentist's contact info should also be available.
 
If you have shadowed for a long time and the dentist had a chance to get to know you, it is best to have a letter of rec that includes number of hours shadowed.

That's what I did. I shadowed for over 3 months and really had a chance to get to know the dentist and vice versa. I even started helping puring models in the lab and setting up/cleening up the room. Since I did so many hours and had a good feeling that the dentist would write a great letter, I asked for a letter of rec that also included # of hours shadowed.

But if you only did the minimum required then just a simple letter with hours will be better.
