Letter of rec comments

Do you need to attach a traditional letter of rec along with the questionaire or is the questionaire alone good enough , do schools really read the comments the writer of the letter of rec submits?

I have noticed that PharmCAS does not really have a "Letter of Recommendation" that needs to be submitted. It is more of a document that is filled out. That kinda took me by surprise.

Actually, they're supposed to attach a letter or place it in the comments section in addition to the form.
