Do letters of recommendations for medical schools generally also come with a requirement for the recommenders to evaluate you on a numeric or some kind of "good", "very good", "excellent" type of scale? What does the usual recommendation entail for the recommender (how involved is it)?

Sorry, but also, what is the timeline from when the medical school (that does not use the AMCAS recommendation format) gives you the format to when it needs to be submitted?
