Letter of Recommendation Question

If my teacher writes a letter of recommendation, what format should it follow ? Is there a standard or something along those lines ? Any help would be great.

If it's for medical school, it should probably talk about your abilities/achievements and how that person believes you would make a good physician. I don't think there's a definite "standard," but most people would know how to write one (unless you're getting one from a family member or something).
