I am pretty sure one of my letter recommender spell my name incorrectly. How bad is it in terms of the overall quality of the letter? Does that makes people think the letter writer doesn't know me well?
My name is a little hard to spell in general...
It's not possible for the writer to correct the error since 1. He is too old. 2. If I ask him it would seem too rude.
I believe the information in the letter is good, just the name.

I don't think it would seem rude to ask him about it. It's probably a really simple error to fix. That's just my personal opinion, though.

I am not 100%sure that he wrote my name incorrectly. It just based on the fact that he never spell my name correctly. And he is also very slow in writing letter, last time the letter took half year to show up in my interfolio.

Well to determine how bad that would actually look, I would ask an someone involved in admissions, like @Goro. I would probably just assume that it was a typo if I were reading it myself.

Chill
I've seen plenty of LOR from less than diligent writers.
Would you regard this kind of letters as "bad letters"? I mean the letter with grammar error or typo even though the information in the letter is good?

Depends on who your dealing with. There was a volunteer activity I has a long time ago with my state medical school. I was meeting with one of the faculty members. She was discussing my letter of recommendation for the activity. She had noted that my name was spelt wrong on the email to her with the recommendation and gave me the stink eye. Then she said that my letter actually had my name correct and gave the look that said "never mind."

Can't blame you for feeling that way.

One of my professors misspelled my name when writing my LOR, I asked her if she could correct it since I didn't want it to seem odd to the person who would be reading it. She told me that they (adcom) don't look at minor things like that. But she still corrected it.
