Hi! I have been observing for a few Occupational Therapists. I know some schools require specific forms but I just have letters stating that I observed there with a signature, I wonder if this would be a sufficient replacement?
 
Hello! I would contact the admissions counselor for the program that you're applying to and ask if that would be appropriate. Since some schools have different requirements, it would be more helpful to ask the program directly so that you know for sure whether you have all the required materials to apply. I would contact the school you are interested in applying to and ask. It doesn't hurt to ask and make certain it is acceptable.
