• date
• written on official letter head
• signature

Am I missing anything?

Should the ID be included explicitly on the letter? I thought that the letter would be linked via ID either using the AMCAS Letter Service or from Interfolio.

It will be linked but just to be safe it wouldn't hurt to put it at the top
