letters of recommendation

hi.
can anyone help me clear up what I need to do for letters of recommendation?

I request them from the people I need.... but who would be the better choice...

if I only took one class with a prof and spoke to him once is that okay? or is an undergraduate advisor okay too? how many doctor recommendations does one need?

who do the schools want mostly?

Any if you don't mind letting me know how long it took you from your request to get the letter then for the school to process and send it off....

From everything else I've read, you want to get the best recommendations possible. If you've only spoken to a prof once, he probably doesn't know you very well, and the letter may read a lot like a form letter (i.e. X got an A in my class, puts him at the top x% of the students, etc.)

I don't know about undergraduate advisors from whom you have not taken classes. Have you done research with him/her? Otherwise it seems sketchy to me, but that's just my opinion.

I don't have any doctor recommendations, so hopefully one needs none, but I dunno...

Do LORs need to be directed to a specific school or person? Can the greeting be to the adcom committee?? I would like to use the same letter for many secondaries? How do I do this?
