What materials/information(resume, personal statement, etc.) is usually given to people who write letters of recommendation? Do you only give them stuff if they ask for it?
Almost no school uses a LOR form, VCU has an optional one but for the most part dental schools expect the professors to have their LOR in letterhead form, signed if submited on paper or not signed if submitted electronically.
I would schedule a meeting with each of your evaluators and let them get to know you better and your goals, etc. Also, hand them your personal statement, resume, and a copy of your transcript.
It just allows them to get an idea of what they'll need to do come submission time, and may help avoid any possible problems that could arise (especially if they aren't very proficient with a comp). Also gives them the opportunity to get an early start in getting the letter file setup.
