Levels of Care?

I've seen a few times where schools state providing different levels of care (primary, secondary and tertiary and sometimes quaternary). What does that exactly mean? Is it simply primary care vs. speciality? If so, what's the difference between secondary and tertiary care?
 
It generally follows the levels of referral/consultation involved in the delivery of care.

Wikipedia covers it better than anyone would be likely to explain here.
