Here goes the process of getting a license for another state. Ugh! Anyway i was wondering how to interpret and answer this question.
Have you ever voluntarily surrendered a professional license, certification, or registration?
I did have a license in Wyoming that I declined to renew because i was not anticipating needing it anymore and didn't want to keep paying the annual fees. Is that "surrendering"? I think of surrendering as more of something you do under some type of penalty, but they do have another question about complaints and sanctions so maybe I am reading too much into it. I will probably contact the board, but have found that the people who work there tend to not want to answer those types of questions. Did I mention how much I hate the application process?
 
Surrender typically means you have an active license, have a board complaint brewing, and say, "never mind, I don't want to be licensed anymore.". It's a way to stop a board from having the authority to sanction you. Usually done by older people, mostly because they didn't keep up on the CEs.

Not renewing is something else.

You can always call the trust for advice. It's "free"/included in your premiums. Or if you are fearless, you can call the board and ask.

That is what I thought too, but it was odd because other licensing apps have included that question with the one about disciplinary actions and suspensions and then had another place to list all prior or current licenses.

Yeah, I think I'll call the trust. It's not like they have had to pay a claim on me or anything so they owe me. 😉

Call the Trust. I’m almost certain the answer is no but best to check with them. That’s what they’re there for.
 
Just wanted to thank everyone for their input and give the follow-up. I did call the Trust and they were very helpful. In retrospect, I should probably have called them for other ethical questions that i have had in the past. Typically I have just asked colleagues these types of questions, but this is a helpful resource that was fairly easy to use and since they are the ones who would be on the hook if I had a complaint or lawsuit, it makes sense to utilize them.
