Any idea what the chances are that I can match derm with a step 1 score of 246? I come from a low-mid tier medical school that has no affiliation with derm residency programs. I've done 0 research in derm but have done a decent amount of research in other specialties. I've always been interested in derm but decided I would wait and see how I do on step before seriously considering it. Considering my score + low-mid tier school, is it even worth giving it a try?

Honestly, I've always felt that derm was one specialty where they really specifically value research in the field and getting to know people in the field. If you truly have 0 relevant research and haven't started creating relationships within your home department, then you really need to commit and start playing the game if you want to have a shot.

You can match. Your step one will put you in the conversation. But everyone else interviewing will have similar scores. Need research, good LoRs, interesting hobbies/conversation skills to stand out on interview day, and strong performances on your derm rotations. Preferably a few strong away rotations at programs which have a history of taking rotators.
