When I talk to patients I always feel like I am missy key things to ask them to put in the subjective portion of SOAP? What do you all suggest?
Off the top of my head I normally ask..
1) How is your mood. Sometimes I ask to rate it as well.
2) How did you sleep
3) How are you eating
4) Any problems you're having or want us to know about.
5) Are you hearing any voices or hearing things you think shouldn't be there?
6) Do you think you're having any hallucinations?
I normally always pay attention to internal stimuli for the subjective portion as well.
