I remember Uniformed services require some LOR checklist or something where you have to write down the names of letter writers.. but not quite sure if that's what you are looking for.

Also, i didnt know you need extra sheet for USC. Is that when you main in individual letters? Just curious.

I know what your talking about. The signed waivers/forms that get provided to the recommenders for them to include with the LOR. Here are my additions.

So does this mean you can't use interfolio with schools that have this requirement? 🙁

I think interfolio would be acceptable because it is an independent letter handling service (whether or not it makes sense). I saw this on one school's website, but check with your particular schools.
