I'm talking about a Letter of Intent (NOT a Letter of Interest).

What do you think adcoms would think of an LOI sent after an Interview Invite but before actually attending the interview?

it would probably just be shoved in your file and will wait for whenever they review your app after the interview.
 
I don't much see the point. You can express how much you want to attend the school at the interview. An LOI before the interview isn't going to change how they look at you during the interview.
 
Going to the interview and seeing the school give you examples to use in an LOI to more convincingly explain specifically why you want to go to that school. I'd find it off-putting to have someone write one before the interview (it has the same flavor as deciding your specialty before you've even gotten into med school).
 
I wouldn't bother sending it now, but rather closer to the time they may make decisions, which will depend on the school. You should definitely wait until after your interview. From what I have seen, Letters of Intents don't seem to have much more impact than Letters of Interests, but if you can put something on paper about why you like the school, you should by all means do it.

You can just tell your interviewer that school X is your top choice. You're pretty much guaranteed to be asked "Why us?" in the interview. Save the LOI for trying to get off the wait list.
