Currently I have around 4-5LOR's; 2 science teachers, 1 non-science, research, & physician.
If I was to get another letter would it be okay to get it from an organization leader? Or would another physician letter be better?

I'm not an adcom, but I think an organization leader would be better than another physician. It would round you out more.

I'd say go with the organization leader. Letters from physicians don't usually carry much weight since they've only really interacted with you in a passive role (shadowing). But a letter from the leader of an organization you worked with could comment on your performance, capabilities, work ethic etc.
