Technically, you can add a fourth letter if you are doing a neuro rotation at the time you submit your application and you want a letter from your supervisor. This is a super Type-A thing to do, and it is not recommended (by me anyhow).
So, to quote Monty Python
"Three shalt be the number thou shalt count, and the number of the counting shalt be three. Four shalt thou not count, nor either count thou two, excepting that thou then proceed to three. Five is right out."
