What are people talking about when they say their deans office hasn't "scanned in their LORs" or "downloaded their LORs"? Don't you submit ERAS and your letters on your own?

No, your LORs must be sent to your Dean's office, including the page where you have either waived/not waived your right to see the letter (downloaded from ERAS website). The Dean's office in turn will upload your LORs onto ERAS after you have started applying to programs.
