low GPA, reinventor advice for this cycle

Hello, all! Longtime lurker looking for advice and school list recommendations for applying this cycle. I'm a former science teacher with 4 young kids, recently finished a DIY post-bac.

GPA: ~2.6. This includes several abysmal semesters from 8-14 years ago, but good upward trend after I finally got my act together. My DIY post-bac GPA was 3.99, consisting of 54 credits of all the pre-reqs and other upper division sciences.
MCAT: 519 (129/129/130/131)
ECs: 1500 hrs as an MA / scribe in a family med clinic (ongoing), 100 hrs as a hospice volunteer (ongoing), 100 hrs tutoring in the science lab at my university, 150 hrs shadowing, minimal research, couple years of teaching middle & high school, 10 years of work experience as a server / bartender (ongoing).

Any insights or success stories from others who overcame low GPAs? I'm ready to apply but would love help narrowing down a school list. Thanks @Goro for the excellent guides and to all the other folks doling out indispensable wisdom on here.
 
OP has little NONclinical volunteering. The schools that are most likely to reward reinvention are service loving.
Teaching pays so little it should be considered an act of public service if it isn’t already 😞
