has anyone heard of these forms, esp. those people from chicago. i just heard of these from some people, but i cannot find anything on them. If they are necessary for residency, can someone please tell me the website in which i can find them. THanks a lot

You're not giving us very much to go on. What is the form for? Why don't you ask your residency director or whoever you would be giving the form to.
