Lutheran Medical AEGD

Anyone applying to the Lutheran AEGD programs? I'm a little confused as to the application requirements. Some program sites list additional requirements, but the addresses listed on PASS don't quite make sense (for example, AZ dental school address for San Diego site). Any one else apply to any of these sites?

Also, has anyone heard anything about any of the program sites? I've found it's hard for find out information about the programs.
