Making Cheque out to Roseman?

Hi, I'm trying to send a cheque to Roseman University for the application I submitted yesterday. I am confused as to who exactly I'm supposed to write the cheque to, since on their FAQ page it doesn't seem to state it explicitly.

Also, for the 3x5 index card with our info on it, can that just be hand written or should I get it printed somewhere?
