UPMC has a fantastic stroke research program, particularly in intervention, where they are essentially second to none. They have a great neuro-trauma unit as well, although up until now they haven't had any neuro-trained critical care physicians (recruiting 2 this year, I think).
Cleveland Clinic is very well-rounded, and a great place to get your residency training, as you will have the chance to do pretty much anything you want for fellowship coming from there. But it is in Cleveland. The Indians are pretty good, I guess, and the Browns are there, which is OK, although the original Browns are now in Baltimore. It is also close to Canton.
Maryland is great for stroke, particularly epidemiology. Brackie Mitchell is there, and he is a pretty legendary stroke epidemiologist. Lots of other very good and personable stroke faculty as well. Don't know too much about their other neuro divisions, but that's just because I'm so narrowly focused.
