Hi. I'm an IMG and I want to do an Internal Medicine residency in US. I'm still not sure if nephrology or endocrinology (like them both; need more research).

I have 2 questions:
-How is the process for applying for those subspecialties? First I do Internal Medicine and THEN apply again for the subspecialty? (in my country there is a defined lenght of time of internal medicine for each subspecialty).
-What do IM residents recommend for making myself attractive for the recidency programs? I've already did a little externship before, and I'm planning on doing some others next summer. Don't know if the externship should be for general internal medicine of a specific subspecialty physician. What else can I do?

I am assuming you know the basics first: USMLE Step I and II, possibly III for FMGs and also step II CS before you even apply for residency. Yes, you first have to apply in Internal medicine, which is a 3 yr residency and then apply for fellowship in endocrine and nephrology (2-3 more years) during your 2nd or 3rd year of residency. As an FMG your task first should be to secure a residency in an academic program or a competitive community program which have a good record for putting their residents in these fellowships. Doing an externship at a place where you would like to do your residency is a great idea to boost your application; you can also do research, etc and try to get a strong letter of rec from a well-known faculty up there. Keep in mind though that nothing can surmount a need for a strong step I and II scores for an FMG so if you haven't already study hard for these.
