i have heard that Maryland & DC residency programs & PD’s / these two states in general are not very DO friendly. Is this true/anyone heard similar? / Why that is?
Looking at a lot of MD residency programs online, I do notice most residents are MDs and IMGs. Do they really take IMG’s over DO’s? Would Maryland residents who want to go back to Maryland residency programs be advised to reconsider DO options?

Do is historically a Midwestern phenomenon, and has spread outwards. Some of these places are snooty and pedigree snoobs, another factor is there is a high density of immigrants anyway in these places that may contribute to fmg pool of applicants.

Depends on the PD and speciality. I've seen Georgetown and GW take DOs in certain specialties but not others. I think Hopkins has a few DOs in their anesthesia program (could be rockstars though) and a bunch in PM&R.

Competitive specialties aside, i think the bigger question here is are alot of DO's even applying to these programs and not getting in? If im an IM/EM/GAS applicant as a DO, am i being discriminated against from GW/Georgetown/UMD or is it that my board score threshold for getting in has to be MUCH higher than my MD counterparts applying(I.E. 230 for a DO vs 220 MD)

most DO's are not from these areas. They end up applying to programs close to their school or where their homes. There is some self selection, but there is possibly some bias as well. There is also a self reinforced bias that just by looking at the roster you dont see and former DO's so you dont apply to these programs. These PD's may not be used to interviewing or ranking a lot of DOs and may not be familiar with the quality of applicants either. I dont think there is data that says DOs have or have not applied to these programs in the past.

Depends on the specialties. There are a ton of IMG dominated IM programs in the greater Baltimore area because there's a lot of FMGs in MD who are looking to retrain. Generally the DMV is receptive to DOs but you're going to really need USMLE scores to have a solid shot.

That’s a big thing I noticed there’s a massive amount of FMG’s in IM here and I’m curious if they really are preferring fmg’s over DO’s. Since when is that ever a thing? I thought DO’s always pulled rank over FMG/IMG’s

a FMG who has >10 years of experience is a real doctor. A first year true intern isn't.

DOs are not always preferred over FMG/IMGs in the northeast. The proponderance of FMGs can probably be explained by immigration enclaves within the DMV area resulting in an influx of immigrants who also happen to be doctors or who send their children back to their home countries to become physicians. When they arrive they spend a large amount of time cultivating relationships with the local residency programs and as a result are selected. There are also no DO schools in these areas so and as stated before DO's were predominately a midwest thing before the last few years, so people tend to train close to home.
