what are preliminaries for in match lists? If there are many students doing a year of prelim, does that mean that they did poorly and couldn't get a full residency?

I'm pretty sure that preliminaries (1 year transitions) are required for certain competitive specialties like anesthesia, opthamology, derm, etc. All the lists I've looked at tend to have these extra years for only certain specialties..

You should look closer into those "prelim" people"

Some of them are doing a PGY-1 before their pgy-2 specialty. Some probably didnt match to their specialty and are doing a pgy-1 prelim year while they reapply.
