In a bit of a bind here guys. I took CS early Jan and then did my match stuff. I received my failing score after the Feb 22nd deadline. I matched at my #1 program on Mar 16th. I retook the test, but don't know what will happen if I fail again. Any suggestions?

1.
don't fail?

2. If you fail a 2nd time, do you have time to retake it (the 3rd time?!😕) and get a score back before July 1?

3. Don't fail.

4. You should probably tell your program director what's going on. I don't think they would want to hear that you failed the 2nd time before hearing about the 1st.

haha...it would be a bad day for PD and you if that happens...

I thought you just had to have a pulse throughout med school to pass CS. Have I been misled?

3% first time failure rate in US AMGs. that's like 500 people/year (18000~ medical students/year).

you need more than a pulse.

now go read.

That's how it used to be. Nowadays they keep adding more and more crap to it. Probably to try and justify the $1500 they rip us off for it. In addition to the travel and hotel cost.

Only if they didn't consent or were under majority age. Many med students learn pelvic exams on paid actors these days.

Yes, but none of the actors at the CS exam would agree to it.

Yup, I learned pelvic and rectal exams on those kinds of "actors" - what a weird job that must be. They must be paid very well...

Not literally, but it may be important for the note.

Oh, yeah, pelvic +/- rectal exams should appear on almost all of your diagnostic test lists for pts with abd pain.

And everyone gets a CBC.

It's just the last 3 you need to worry about.

It's supposed to get harder next year I think. I think you will have to justify why you are ruling things in and out of the differential. I found it a little stressful and can see how people fail it.

We did pelvic/rectal and male genital exam on actors this year. I asked them about it and they get around $40/student for the male exams, and $60/student for pelvic. One of the standardized patients was from Michigan or Minnesota, can't remember (is there a difference? They're both tundra as far as I'm concerned). He said every fall he loads up in his RV and heads down to Florida doing rectal/genital exams along the way for extra cash. He was around 60 and has been doing it for years. Strange fellow, but very nice.

Yeah, that almost goes without saying.

They sometimes also list the rectal/pelvic exam findings IIRC for you to formulate a differential/plan.

OP, why exactly did you fail the first time? Literally the only reason I ever hear of people failing CS is because the SP could not understand their English because they're fresh off the boat. I know they're changing the exam to make it "harder" but if you've gone to a school with good clinical teaching, OSCEs, etc then it really shouldn't be a tough exam.

I've heard of folks failing because they didn't do all the niceties like telling the patient that they were going to untie this, listen to that, didn't give the patient a sheet to cover themselves, didn't wash their hands, didn't make the comments that feigned empathy, and didn't counsel patient to avoid smoking, drugs, alcohol, unprotected sex.

75 year old male: so this burning down there I have ... could it be from the unprotected sex?
me: YOU STILL HAVE SEX?

I think I may have gotten docked a few for that...

Think you still did better than me...

I just gave him a high five and said, "Nice!" in a Borat accent.
