Someone please help me! I am a DO student who applied late to PM&R programs. I only interviewed at advanced positions.

I did not apply to any prelim/Transitional year programs.

When I go to NRMP side to create my rank list, I ranked only the advanced positions and have no associated prelim/ty program with the advanced position.

Here are my questions:
1) Can I match advanced programs w/out applying and ranking any prelim/ty programs?

2) As a DO who only took COMLEX, can I match at MD prelim IM or surgery programs via the scramble? Do they accept comlex?

3) Am I allowed to do a DO prelim instead of MD prelim? if the answer is yes, then if I scramble to a DO prelim, will I lose my advanced MD positions since I matched DO prelim program?

4) Which is better to do - Transitional year or Prelim? which is more easier to scramble into?

Just scramble into an AOA TRI come Feb 10. It's acceptable by programs.

You'll be fine.

What have you been busy doing the last few years that has left you so poorly prepared and uninformed?

I think u can probably match into a md pmr and then jusst do traditional one year through do. the majority of them end up going unmatched anyways. can anyone second that?

1- yes
2- yes. many do, probably most
3- currently, yes. there is some concern you may not be able to do this in the future, or that it might affect your fellowship opportunities, but i doubt it. you can take any PGY1 position without losing your ability to participate in the match for a PGY2 spot which begins one year later.
4- there is no one answer for this. long story short, either one will do. it's up to individual preference.

Generally the TRI's are easier to get through with more electives and less call but the Prelim Medicine might be a better education in terms of floor medicine and such. Since you are scrambling, you might have to take whatever you can get, especially if you are restricted to a certain location. Either would be fine though.

Also, you know you will have to scramble after the DO match but before the MD match right? I would not wait for the MD scramble to try and get a internship since the DO ones will fill up (at least the nicer ones) and the MD internship scramble is not so easy for DOs (you might get stuck doing a surgical prelim).

wont that jeopardize your chances to match into a categorical program or a different speciality?

I suspect he was interested in a completely different specialty that didn't require intern years so never had to research it.

Yes. If you match or scram into a PGY1 spot with the DOs you will be excluded from any PGY1 spots in the MD match, including categorical PMR residencies or other specialties. Since you said "I ranked only the advanced positions" I don't think this will be a problem, but if you also rank a back up specialty this might come into play.
