matching/ranking residencies process

forgive my ignorance-- but i am confused as how to exactly the residency process works, and by process i mean applying for residencies?

what does everyone mean when "they" say a student should begin ranking by the end of the 2nd/3rd year?

What does it mean to scramble? what does this entail?

When a student "ranks", (dont know exactly what that means), residencies, do they rank 2,3,4, how many programs? Is ranking like filing for your choices? If you don't match with your first choice, do you get bumped to your second choice for residency and so forth?
