Is it possible to receive and MD, and not go into residency? I mean for all those hospital executives/CEO's, a lot of them have MD's. Question is, why get an MD, and struggle through a residency, when being a hospital executive is the only thing you wanted to do in the first place? Why not just go straight for the MBA?

Nothing requires you to do a residency. A small fraction of people don't match, another small fraction have other interests that either keep them from residency or make it less important for them. Most people who end up being hospital executives didn't know in their 3rd year of medical school that is what they would end up doing.

as mimelin notes, residency is not required if you don't want to practice clinical medicine. But the MD degrees is worthless without residency in most instances.

Most hospital CEOs are not physicians but rather businessmen with MBA/MPHs. There are the occasional MD/DOs and these are typically people who have left clinical practice for whatever reason. But there are a few who realize they don't want to practice clinical medicine (we see a few every year on SDN) and look for something right out of the box - if they knew before medical school, they would have chosen another route.

The MD is only valuable if you have the clinical experience to back it up. No hospital administrator who has an MD just has the degree. while most if these jobs go to MBAs or JDs, there is certainly a sentiment that someone who has actually worked at an institution in a clinical capacity can bring a certain kind of insight, so a number of people with MD/MBA and RN/MBA also get considered for these spots AFTER they have some experience as a clinician. So no, the MD without residency is worthless even for these paths.
