md/mba programs that don't require gmat?

Anybody know of any md/mba programs that don't require a gmat that also doesn't require a separate application for the md and business schools? I know jhu and tufts are examples of that but just wanted to know if there were any others.

Tufts requires you to take the gmat before matriculation, but doesn't require it when applying. So I'd be ok with schools like this as well. Many thanks!
