Do you see Medicaid patients under direct supervision?

If you work in an outpatient physical therapy clinic, do you see all of your Medicaid patients (including HMO Medicaid replacement plans) under strict direct supervision and one on one? I know they say they follow CMS guidelines but they also say that and then the rules differ wildly in a lot of other areas from what Medicare will allow. I am writing this to see what is the norm because I don't know many outpatient PT's outside of a nursing home setting where everyone is one on one regardless of payor.

I work for a rural hospital, mostly outpatients, and I see all of my patients one on one.
