  1. Which are the top medical retina fellowship programs?
  2. What are key considerations? I know believe some require medical retina fellows to take retina call along with the surgical fellows, while others do not.
  3. Any insight into which allow you to continue to operate during fellowship? I've heard some actually allow you to do cataract surgery, which is huge plus for me as I think about maintaining my surgical skills. I would like to have a "comprehensive-plus" practice in the future.

What is your exposure to medical retina like in residency? As comprehensive plus you will probably be dealing with mainly diabetic retinopathy and AMD. A medical retina fellowship may not be necessary. I do a ton of medical retina in my practice (mainly diabetic retinopathy), and I feel that my exposure during residency was plenty to get started. As long as you put in work to stay up to date this is very doable without fellowship.
 
If you're going to do medical retina life long, absolutely 100% worth doing a fellowship. Its just 1 year and you get exposed to a lot of different variations in the day to day management of common/uncommon retinal conditions. Even if you had great exposure to retina during your residency this 1 year will prove to be very helpful. You will also be able to practice medical retina in desirable locations without being frowned upon unlike comp specialists dabbling into medical retina.
