Memranous Glomerulopathy vs. FSGN

I was doing questions on Kaplan qbank and came across a typical membranous glom question, adult with proteinuria etc.

the answer choices were many 2 being Membranous Glom and FSGN, so based on w/e is written in FA and books i went with Memrbanous but the answer was FSGN. Logic being that over the last few years FSGN has surpassed Membranous is therefore more common.

Now my question is I cant recall ever running into this particular question on UWorld where it mentioned FSGN being more common. In Uworld its still Membranous ( as far as I recall)

And in Kaplan it says its a "new question" so come exam time, if same scenario repeats what should I go with? Can i really trust Kaplan in terms of its "new information"?

cant wait to be done with Renal, hate this chapter! :laugh:
 
FA: membranous GN is the most common and FSGN is the most common in HIV patients
FA Step 2: membranous nephropathy is the most common in Caucasian adults

Goljan: FSGS is the most common and membranous glomerulopathy is the 2nd most common

Pathoma: FSGS is the most common in Hispanics and African Americans and membranous glomerulopathy is the most common in caucasians

My professor's notes (one of the few that I trust): membranous nephropathy used to be the most common primary cause of nephrotic syndrome in adults, and some references still indicate that it is, but the incidence of FSGS has been increasing and in many series FSGS has overtaken membranous nephropathy as the most common cause of nephrotic syndrome in adults. I really don't know which would be the "right" answer if the "boards" asked about that (I hope that they won't).

That should clear everything up. 🙂

I forgot to mention that diabetes is the most common cause overall.

yup thats the explanation I got from kaplan, membranous USED TO BE but FSGS has taken over. Either UWorld hasnt caught up with it or Kaplan is too advanced for our own good lOL

If this happens on the "boards" i will pray to the NBME gods to point me to right answer!
 
i seem to remember someone asking this question to Dr Sattar on pathoma's facebook page and him saying if it shows up on the boards to pick FSGS.
