I have come to the conclusion that I am a midwest guy, so I am only going to apply to this region. Here are the programs I have come up with so far:
Northwestern
University of Chicago
Washington University
Iowa
Rush
MCOW
Michigan
Mayo
Vanderbilt
Indiana
UIC

Am I missing any good programs, or does this look like a pretty good list of places to apply in the Midwest?? I was hoping to get around 15-20 programs on my list. One other question about applying to transitional/prelim year programs.....Do they match the transitional years at the same time as the advanced positions? It seems that it would be advantageous to have a separate match for the prelim positions so that a candidate could know what city they were going to and then rank the corresponding transitional year programs higher.

From what I understand your match list goes like this:

1. Anes 4 yr program (don't need to list any transitionals for this one)
2. Anes 3 yr program
2a. Prelim yr 1st choice for this program
2b. Prelim yr 2nd choice for this program
2c- whatever, rank prelim years that you'd like if you match at 2
3. Anes 3 yr program
3a-whatever, just like number 2

You list the transitional years as a sub-list of each advanced anesthesiology position. This way you can rank the prelim year in one city first for the 3yr anesthesia position in the same city, or however you want to rank them. Of course you don't need to list any if you also rank categorical (4 year) anesthesia positions.

Have you thought about MU at all (Univ of MO. - Columbia)? It's a small program, but very "hands-on" from what the residents tell me.

I would add UW Madison to this list. Very friendly program with a good reputation. I really came away with a good impression from interviewing there. I think they're also getting a children's hospital soon, if I remember correctly, so maybe the peds experience would get better. They send their residents down to Northwestern for OB experience.

Please disregard the earlier post about UT Houston. As a CA-1, my expereince has been great! Everybody here is great to work with. Though there are changes being made, they were changes that needed to be done. Our new chair Dr. Arens is a pretty big name in the anesthesia world and I have all the confidence in him that he will make our program even better than what it was before.

May I suggest Cleveland Clinic...don't know much about it personally, but the name comes up in "top midwestern programs" discussions...

P.S. Since when is Vandy in the midwest?

Might as well put Loyola in Chicago to your list to complete the Chicago scene. If you're looking to get to the 15-20 programs to apply to, and this is smart, try Henry Ford in Detroit as well. Detroit sucks, but the training seems awesome. I've met some attendings who trained there and have been very impressed. Pittsburgh may be stretching it for midwest, but it's a great program as well.

As for the transitionals, the only 'separate' match is called the scramble. Otherwise you rank your 3 year programs with a selected transitional year. Assuming you don't want to move, you interview at X and Y transitional programs in, say, Chicago. You rank Northwestern, UChicago, etc. and next to each rank X and then Y.

If you're willing to move, I highly recommend checking out the Michigan State transitional programs in Kalamazoo and Grand Rapids. West side of the state, great perks and benefits, and the lifestyle simply can't be beat.

Cleveland Clinic is a massive massive residency. Id probably check UWisc ahead of this because of the personal anonymity (which my be beneficial for some). It is a good program however.

I agree with throwing Loyola into the mix. If you are here you may as well check it out. Has a family feel to it.
