Besides what are under "accepted student checklist", what else are required?

Are medical history and immunization forms the only two other forms required.

Do I just print these two, fill out the forms, and fax them to the school?

I know you have to get a physical as well. You also need to apply for financial aid.

Well if you can pay cash, you do not need to apply for F.A, you just present the check.
